This practical one-day workshop develops hands-on skills in problem identification and root cause analysis – the techniques that enable teams to move beyond repeatedly resolving the same incidents to actually eliminating underlying causes.

The workshop covers the key RCA tools and techniques: 5 Whys, Fishbone/Ishikawa diagrams, fault tree analysis, timeline analysis, and structured problem statements. It is highly practical, with exercises and case studies designed to build confidence and competence in applying RCA techniques to real situations.

While rooted in ITIL® problem management principles, the workshop applies equally to operations, manufacturing, and any environment where structured problem solving is needed.

By the end of this workshop, you will be able to:

  • Write a clear, structured problem statement
  • Apply 5 Whys analysis to identify underlying causes
  • Use Fishbone/Ishikawa diagrams to map potential causes
  • Apply timeline analysis to sequence events and identify contributory factors
  • Select the right RCA technique for different types of problem
  • Document findings and communicate root causes clearly

This workshop is suitable for:

This is a workshop-format course. A certificate of completion is provided. No formal examination.

Inspired by this radiant sight, one of the founders suggested the name “BrightOak.” The name embodied the brilliance of innovative ideas and the steadfast strength of the mighty oak, a perfect metaphor for their consultancy.
